Although "scapholunate dissociation" is a descriptive term, it is used synonymously with its primary pathophysiologic correlate, scapholunate interosseous ligament (SLIL) injury. Although injury to additional ligaments is present in scapholunate dissociation, the SLIL is the major target of surgical reconstruction.

Scapholunate dissociation is the most common and most significant ligamentous injury of the wrist.[1][2] Scapholunate instability is the most frequent pattern of carpal instability occurring separately and as part of other wrist disorders.[3] It results from the relative instability between the scaphoid and lunate bones secondary to the injury of scapholunate ligament and generally

Management of scapholunate instability is both complex and controversial. Therapeutic options range from splinting and casting or arthroscopic treatment to ligament reconstruction using autograft tendons. Nearly 40 years after the seminal work of Drs. Dobyns and Linscheid, the results of surgery remain unpredictable. The wrist is complex joint consisting of 8 small bones, connected to each other with many strong ligaments.

Although good to excellent results have been reported using a variety of surgical techniques, they have often not been confirmed by other authors.

Although injury to additional ligaments is present in scapholunate dissociation, the SLIL is the major target of surgical reconstruction. The scapholunate (SL) ligament runs between the scaphoid and the lunate and provides crucial support to allow appropriate carpal mechanics at the wrist during motion and use of the hand. Injury and tearing of this ligament can occur with trauma or as a progressive degenerative weakening of the ligament. Diagnostic X‐ray examination showed severe degenerative joint disease of the left wrist with scapholunate dissociation, scapholunate ligament tear, and avascular necrosis of the lunate (Figure 1).

In this chapter on scapholunate ligament injury, we first describe wrist anatomy and biomechanics so that the reader can then have an understanding of the pathomechanics of scapholunate dissociation and its classification.

Scapholunate dissociation treatment

Scapholunate (SL) instability is the most common dissociative carpal instability condition. It is the most frequent cause of wrist osteoarthritis, defined as scapholunate advanced collapse or SLAC wrist.

If you had a recent scapholunate ligament injury, the early goal of treatment will be pain control. This may include wearing a wrist splint or cast, decreasing activity and resting the wrist.
